Position: LVN - The Fountains

Experience: Graduate of an accredited school of professional nursing.  Texas State Board of Nursing LVN License.  Current BLS certification

Skills/Duties:

  • Must administer and be current, confident and knowledgeable of medications based on age and clinical condition of patient

Position: Unit Assistant

Status: Full-Time

Education: High School Diploma or GED.  Completion of a state approved Monitor Technician course

Skills/Duties:

  • ​Perform cardiac monitoring duties under the supervision of the registered nurse
  • Communicate relevant information appropriately
  • Demonstrate accurate and timely documentation of patient cardiac rhythms
  • Perform clerical duties on unit
  • Coordinate the flow of selected information within the Med-Surg Unit and between the Med-Surg Unit and other departments of the hospital to ensure efficient communication of patient needs and timely compliance with medical and nursing orders
  • Responsible for the processing of all physicians' orders using computer order entry and MARs
  • Act as a central resource regarding the procedures used to obtain services for patients

Position: Medical Assistant - West Columbia Health Clinic
Status: Full-Time - Non-Exempt
Education: Medical Assistant Certificate, BCLS certified from the American Heart Association
Experience: 2 years prior medical experience
Skills/Duties:

  • Collect blood and prepare laboratory specimen and/or perform basic point of care laboratory tests as well as ordering needed laboratory supplies

Position: Dietary Tech - The Fountains

Status: Part-Time (Saturday and Sunday) 6am - 6pm

Education: High School Diploma or GED Equivalent Required

Experience: Prior work experience in the Institutional or Commercial food service area.  Food handler's certificate.  Must meet all local and state requirements regarding food handling.

Skills/Duties:

  • Prepare and cook food for residents and guest
  • Review menu to determine types and quantities of food to be prepared
  • Plan cooking schedule so that food will be ready at specified times
  • Assist residents with trays to the dining room and residents areas as needed
  • Clean cooking equipment and maintain high sanitary standard in the kitchen
  • Assist residents with activities of daily living as needed
  • Set dining room for meals and clean dining room
  • Perform other duties as assigned

Position: Clinic Supervisor

Status: Full-Time

Education: RN or LVN current license by the State Board of Nursing.  BLS certified from the American Heart Association

Experience: 2 years' experience working in a clinic environment

Skills/Duties:

  • Contributes to the assessment of patients by collecting data for analysis by an RN, PA, of Physician
  • Administers medications and observes patients for adverse reactions to medications or treatment
  • Coordinates TVFC program in the clinic and prepares monthly reports
  • Educates patients/families about disease treatment plan including self-care3. post-discharge, preventive and holistic health needs, available resources, and follow up care
  • Documents delivery of healthcare and nursing process in EMR in accordance with policies and procedures in a timely, accurate, and consistent manner
  • Reports abnormal health data to the provider
  • Calls patients with diagnostics’ results after the provider has commented on educating them and answering questions
  • Calls patients for care coordination and chronic care management to assist with the attainment of a healthy  lifestyle

Position:​ Patient AR Specialist

Status: Part-Time (20 Hours)

Education/Experience: High School diploma or equivalent.  Three years' experience in healthcare billing.

Skills/Duties:

  • ​Monitors and follows up on claims in the 91-days and older patient account receivable categories for timely filing and payment, determine the reason for any delays and takes appropriate action for resolution
  • Monitors denied charges in the 91-days and older patient account receivable categories and take appropriate action to immediate appeal and mitigate final denial risk
  • Prepares insurance claims for billing, if not processed within the first 90 days
  • Receive and answer billing related inquiries, prepares billing report and handles financial transactions related to accounts in those categories
  • Works in partnership with other Billing Office team members to achieve PFS goals and objectives
  • Assures all claims are accurate and follows generally accepted insurance claim filing guidelines and those specific to an insurance company
  • Receives insurance remittance, advises electronically and either auto-posts or posts manually
  • Process credit balances timely to meet State of Texas Prompt Pay Act requirements

Position:​ Resident Care Attendant - The Fountains

Status: PRN

Education: High School Diploma or GED required.  Valid Texas Driver's License with an acceptable record. Must obtain and maintain BLS, CPR and AED training.

Skills/Duties:

  • At least one year of experience working with the elderly in a residential setting

​Education: Current Texas DSHS EMT - Paramedic Certification or License.  Current American Heart Association BLS Provider Required.  Current Texas DPS Driver's License (Class C).  Incident Management Systems ICS 100, 200, 700, and 800.

Skills/Duties:

  • Initiates patient treatment and care utilizing initial protocol treatment based on patient condition.  Conducts basic and advanced life support skills.
  • Responsible for safety decisions involving patients and emergency vehicle.
  • Drives emergency vehicle to and from calls to transport patients in a safe and prudent manner using the most efficient route to ensure the quickest response times.
  • Assist in carrying patients by stretcher or other devices to an emergency vehicle and secure patient inside the vehicle choosing the safest means of transport.
  • Observes and keeps records regarding symptoms of patients.
  • Communicates information to other members of the healthcare profession, patients, family members, and other responders as necessary per HIPAA standards.

​Position: Maintenance Tech

Status: Full-Time

Experience/Education: 6 months of related experience. High School Diploma or Equivalent

Skills/Duties:

  • Electrical repairs, light carpentry, light plumbing, painting make ready, use of hand tools and mathematical skills.
  • General maintenance duties of facility upkeep internal and external as well as in response to Maintenance Service Requests under the supervision of the Maintenance Technician Supervisor.
  • Replace and repair lighting of various types, plumbing fixtures, kitchen appliances (commercial and residential).
  • Inspect and maintain essential operations equipment to include HVAC systems, compressors, generators, life safety equipment and other equipment according to manufacturer's recommendations for SCH procedures
  • Perform urgent corrective, routine and requisitioned repairs as required.
  • Work effectively with/without supervision both independently and as a member of a team.

Position: Infection Control/Employee Health

Status: Full-Time

Education: Current Applicable state RN license. BLS, ACLS, PALS.  BSN in Nursing preferred.

Skills/Duties:

  • ​Responsibilities include the identification, investigation, reporting, prevention, and control of infections and communicable diseases within the organization, including both healthcare-associated infections and community-acquired infections.
  • Knowledge of CDC guidelines and definitions of healthcare-associated infections (HAIs), accrediting organization standards, federal and state regulations, and national guidelines.
  • Demonstrates comprehensive knowledge of the infection control process in the healthcare setting.
  • Demonstrates knowledge of risk-stratified statistics as applicable to trending healthcare-associated infections (HAIs).
  • Completes required CMR reports to the Public Health Department in a timely and accurate manner.
  • Maintains a log of incidents related to infections and communicable diseases, including healthcare-associated infections (HAIs) and infections identified through Employee Health.
  • Designs and implements new methods to improve the efficacy and outcomes of the Infection Prevention and Control Program.
  • Communicates infection control activities to Administration, Performance Improvement Committee, the Medicine Committee, the Medical Staff and MED via the Infection Control Committee minutes and/or Infection Control Committee Chairperson.
  • Prepares statistics and other pertinent data and reports to the Infection Control Committee at least quarterly or as situations arise.

Position: Ultrasound Technologist

Shift: PRN Available

Education: Completion of a two year approved School of Radiologic Technology.  Valid Texas State Radiology Registration.  Registry by the American Registry of Radiologic Technology (ARRT).  Current CPR/BLS Certification.  Certification by the American Registry of Diagnostic Medical Sonography.

Skills/Duties:

  • The sonographer has a basic understanding of human cross-sectional anatomy, pathology, and medical terminology.
  • Cleans and disinfects the ultrasound equipment and probes.
  • Ability to integrate diagnostic sonograms, patient history, and medical records, and adapt sonographic examination as necessary.
  • Maintains certification and registration as appropriate.
  • Explains procedure to patient, assists and transfers patient to table, positions patient for accurate scanning and assists patient at end of procedure.Complies with all policies and procedures.

Education: RN Licensed with Texas State Board of Nurse Examiners, ACLS and CPR required

Position: Clinical Laboratory Technologist/Technician

Experience: Five years general hospital lab experience preferred; Cerner EHR, a plus

Education: Degree in Medical Laboratory Technology or with equivalent education, ASCP Certification MT/MLT required

Skills/Duties:

  • Properly collect, process and store specimens; evaluate specimen acceptability for testing
  • Technical ability to perform all laboratory tests according to CLIA standards, including Blood Bank, Microbiology
